Indications and Dosage
Oral
Productive cough
Adult: As effervescent tab: 50 mg bid. As syrup or solution (containing dried ivy leaf extract 0.7 g/100 mL): 5-7.5 mL bid or tid.
Child: As effervescent tab: 6-11 years 25 mg bid; ≥12 years Same as adult dose. As syrup (containing dried ivy leaf extract 0.7 g/100 mL): 2-6 years 2.5 mL bid; >6-12 years 5 mL bid or tid; ≥12 years Same as adult dose.
Contraindications
Children <2 years. Pregnancy and lactation.
Special Precautions
Patients with gastritis or gastric ulcer. Treatment with a cough medicine in children (especially under 6 years old) should be considered carefully due to potential risks and limited evidence on efficacy.
Adverse Reactions
Gastrointestinal disorders: Nausea, vomiting, diarrhoea.
Immune system disorders: Allergic reactions (e.g. skin rash, urticaria, dyspnoea).
Monitoring Parameters
Monitor for signs or symptoms of dyspnoea, fever, bloody sputum and other persistent symptoms.
Overdosage
Symptoms: Nausea, vomiting, diarrhoea, excitation or agitation. Management: Symptomatic and supportive treatment.
Action
Description:
Mechanism of Action: Hedera helix (Araliaceae) contains saponins that have mucolytic, spasmolytic, and bronchodilatory actions. Based on studies, the saponins found in ivy leaf trigger responses in the gastric mucosa. Hence, activating the gastro-pulmonary mucokinetic vagal reflex which stimulates the bronchi to produce a watery fluid and induce expectoration.
